I'm running this one on my 82 22R (propane conversion) with a flowmaster 40 with all smog related components removed (in Alabama :) ).  I will tell you that I noticed a rather large seat of your pants increase in power.  Though that could be b/c the truck actually has a proper exhaust now.  Before, it was just the cracked manifold and the original cat w/ no muffler.

Just the pro header, the formula header doesn't fit very well, from what I understand, unless you're building a formula toy.  The point is, that if the pro header does that well, the formula should be even better due the less bends in the tubes.

Definitely noticed more on the low end.  I'm running 33" tires on 4.10 R&P atm and I regained part of 5th gear back.

As far as loudness, depends on muffler, but no, mine's not very loud.  Like I said, mine's running the pro straight to a Flowmaster 40 and then pipe back to just past the rear axle and then turned down.

I have one of their long tube headers from about 4 or 5 years ago. The header itself is very nice. I recently had a part break on an extension piece that came with it, although I am told that they no longer make them that way so I guess it doesn't really matter. I had mine running into a flowmaster 50 series, which is supposed to be quieter than the 40 series. It was still louder than all get out. I recently ordered a Dynomax super turbo muffler to go on it to quiet it down.

My LC header and flow master 40 were stupid loud. More recently got a magnaflo the tone is lower and more tasteful.

The street, it was the only carb legal one they offered. It's obviously a little louder but well worth it, no regrets.

I also have the carb legal LC header. when i go to get it smogged all i need is the tag on the header right, no paperwork or anything? i bought it from a friend for $150, not bad for a $450 header Lol.  I havent noticed too much of an increase in power, although i havent installed the air injection kit yet, and ive heard it adds power compared to the block off plates. She probly just needs to breath more, then ill feel that extra kick from the header

C.A.R.B. approved headers will also have the O2 sensor located in the section of tubeing after the header collector.  most non smog legal headers have them mounted up on one or two of the header tubes(easy catch for a smog tech).
