It looks real to me. I've seen them all the time, way back when the whole minitruckin days were big. And a guy I know did a dually conv. on a Nissan and its just a huge frickin spacer and special offset wheels. But with today and everybody that can think they can weld, I've seen some bizarre stuff when I worked at the tire shop.
