So when exactly did the bad shaking start? Also how much movement are you talking about in your flange?   There shouldn't be any movement.  The flange still moves side to side after you tightened it back down? With your driveshaft having a dent in it and your transfercase being able to move side to side I'm thinking your rear main shaft bearing went bad on your transfer case.  But that's with me not actually looking at the truck.  I would start checking things from the easiest to the hardest.  I would check that all your suspension bolts are correctly torqued, i.e. u-bolts and shackles.  Also, make sure the new drivshaft is in series.  Meaning make sure the caps on the u joints are in line.  There is a good write up of going through transfer cases on marlins website. http://www.marlincrawler.com/tech/guide/4701-gear-install
