Possible defect but most of the time its just a fluke deal.... Running a tire at too low of a pressure or overloading it can cause it.
Heck a kid in high school jumped a cressida and upon landing separated all 4 tires at once.... They didn't explode but all 4 of them were out of round and bulging from the separations...
