Loose pinion washer

Any answers to the post by 4RUNNERJADE ?
He posted it to the TECHNICAL ARTICLES section,

WHAT WOULD CAUSE BOTH THE PINION NUT ON THE COPANION FLANG AND ON THE PINION NUT TO STILL BE TIGHT AND THE WASHER UNDER IT TOO BE LOOS?


I will have to ask Marlin on this one.
Is the pinion the right one for the differential?
Is the flange the proper flange?
Is the driveline the proper driveline?
