xfer output flange rear drive shaft

Hi guys,
I had the 30mm nut on the rear driveshaft output flange unsrew itself.  I got very lucky that the drive line didn't drop out.......just thinking about it makes my hair stand up.  It striped out the flange and the inside threds on the nut.  Doesn't look like it damaged the output shaft.

My questions are:  Is this a normal thing on the toys"????
                          How can I stop it from happening again????
85 flatbed, 22RE,Downey air, redtop, dual 2.28 crawler, high steer, Marlin clutch, 5.29's, ARB rear, lockright front, Warn hubs, wrangler 4" front-skyjacker 4" r-springs w/6" shackels, flip kit, 35x12.5x15 MTR's, sliders, belly, exocage.

Was the nut staked? I've never had it happen.

Yep you need to stake the nut, look on the shaft you will see an area where there is like a groove, just take a punch and BFH after you torque the nut and smash the edge of the nut into the groove.

Yes it was staked and it looks like there was permatex on the on the threds.......man that scares me.....gonna be checkin the @#&*(&^%$#@ thing every week now.
Gotta get me a hoop too.

Never heard of that happening, was probably just loose and never checked.

Make sure to check the u joints while you're working on it. The one I saw come loose was due to a u joint binding and vibrating.
