Happened about half a dozen times or so and once on a hill...not fun.

Anyone know what the problem is and how to remedy it?  Or a place to look for help?

Heavy duty detent spring will solve the issue.  I had that problem with mine awhile back.  The HD kit from Marlins cured it.
Full hydro, 186:1 with an auto and 44's what could go wrong??

Also, what gear set are you running? Some of the early low gear sets had problems with this. The answer was they didn't have a backwards taper where the sliders locked onto them so they would slip out of gear. There is a writeup on this in the Marlin comparo thread that Big Mike did  a while back.

Also check for floor boot bind.
We have had a lot of problems with the handle being in a bind when shifted into low range.

Check for broken motor mounts, loose bellhousing bolts, rusted or frozen rear driveline slip yoke.......
