Throwout bearing failure

Anyone know what might have caused this or had this happen to them? Bearing started squealing one day while driving. Dropped trans to inspect and found this. Replaced clutch/flywheel/rm seal and pilot bearing too. No issues. Go to take it for its first drive and the same squealing sound is still there. Only when clutch petal is being pressed or let out. No sound when fully pressed or in gear. It's tough to think the replacement bearing was bad too. Spun freely after in installed on input shaft.

Truck is a 1987 pickup 22r 4x4

I have no idea but holy crap that's the worst throw out bearing I've ever seen.
1981 Pickup - 35s, 5.29s, W56, dual ultimate, e-locker front, grizzly rear, 22R stroker, 3 linked on airbags

It pretty much crudely machined itself down on the pressure plate diaphragm  and now the new bearing is doing the same.
