Is it the adapter made for the 4x4 tranny or does the output shaft come with it?  21 or 23 spline?

yeah its all 21 spline. The adapter is for a Th350 4wd. The shaft for the tranny doesn't come with it. I do have another th350 tranny that the back of it is broke off but the shaft is still fine. If you want that with it you can have it. but you have to take the whole tranny
