advanced adapter 2nd t-cass

SO what I'm I doing wrong that is causing this





This is the second time that the bottom bearing on the front case has given out. I'm running a stock 22re, 5.29s, duel cases with stock gears, but the front case bottom bearing keeps going out. My adapter case is one I bought from allpro 3 years or so ago. It an advanced adapter, I think it's like the MC5. Thanks for you helpfull input.
