I need to id this 5 speed transmission. It came out of my dads pickup i think it was a 1990"s model.

Looks like a W58 to me.
1981 Pickup - 35s, 5.29s, W56, dual ultimate, e-locker front, grizzly rear, 22R stroker, 3 linked on airbags
