dude , that axle flange is bent
being hit hard enough to bend the wheel, yep its tweeked,  you could use a dial indicator but it probably wouldnt show more than a few thousandths :.order:
just replace that axle  or take it to a machine shop and have them remove the studs and turn the flange back true :driving:
