My 89 4Runner has a 22RE with a mid range cam and essentially stock exhaust on 35 inch tires and 4.88 gears.  It turns them just fine, but I live at sealevel.  Crankycrawler also runs 35 inch tires on his 84 truck with 4.88 gears in Colorado.  His engine is pretty much stock if I remember correctly.

If you get the wrong header you won't ever see a real difference.  Headers can improve air flow but generally the performance inprovement is at very high rpm.  This why you see long tube headers on muscle cars, the longer the header the lower the rpm that it benefits.  If you're running a header I wouldn't expect a mid range cam to be a good match to maximize on the combination.

I think 5.29s should do great.

I believe that 4.88 gears with 34 inch tires works out to be the equivalent of the stock gearing and tire size.  My 4Runner's speedometer is very close with 35's and 4.88 so going with 5.29 gears would be just a little lower that stock gearing if you had 35 inch tires.

I have 4.88 gears in my 83, which had a 22r, with 35" tires.  It had good acceleration until about 45mph then it was a dog. Also, once you start going up hills its all over, the speed drops quickly.  If I did it again I would go with 5.29's.  It was that way at least until I finished my 3RZ swap, now I have no problems with power.

Dual transfer cases do not affect road driving at all. Put 5.29s in your diffs and forget everything else. Header and cam will only add a few ponies but will cost you hundreds.
