As long as the manifold is not warped too bad you can "true it up" with a hard flat sanding block.

The block has to be long enough to go completely across all 4 ports. Use coarse emery cloth or sandpaper. You can also just move the manifold across the sand paper on a flat steel work bench. Sort of a poor mans mill. Moving it in a figure 8 pattern keeps it pretty even.

I use this method on intakes and even deck surfaces to remove gasket gunk and avoid leaving an irregular gasket surface that could leak.

Just a quick update: Besides the exhaust leak, which will be fixed soon, everything seems to be running great. I have almost 300 miles on the 4Runner since replacing the HG. I am very happy about doing this myself. Saved a load of cash and it seems to be operating fine.
