22re startup noise

ok heres the deal, '91 22re,140,000mi,super clean doesn't leak a drop, oil looks good, coolant looks good, good oil pressure,good compression,plugs all look good,timing chain guides look good,oil pump bolts in correct holes,tensioner seems to move freely,but chain seems real loose,valve springs/valves/rockers look good,adjusted to .07 and .11,blow-by not excessive,doesnt burn oil,passes smog,runs ok but seems to miss a little from about 2000-3000 rpm when not under a load,but makes a knarly kocking or rattling noise on startup.?. i have owned my share of 22r and re powered rigs and this is not just the normal noises.
what do you think?

Has the timing chain ever been replaced?

The single chains stretch quite a bit with age, and the tensioner will wear out faster than the earlier double chains.

And the guides could be trashed lower than you can see.

i dont have any idea if the chain has ever been replaced.
i have had a 22r in the past that had no guides left and it didnt make near as much noise as this thing is making.
i'm just trying to see if im missing something before i go tearin into it.

i dont know if wrist pin needle bearings would explain these symptoms...or a good way to check them without a full tear down..i would probably opt to just swap in a reman short block...but i dont want to get ahead of myself.

Does it sound like a diesel?

Mine does that in winter upon first startup..............

kinda...like a 6bt cummins on an either startup in -10 degree weather
