Marlin and all-pro offroad are the 2 main bolt-in cages that come to mind.  If you have the tools/ skills, the best fitting in-cab cages ive seen are custom built from scratch, that way you can tailor it to suit your needs...

I would build/have one built for your truck. Keep it tight and tie it into the frame.

You're going to be looking at $80-$100 an hour for labor most likely.  Probably want to use DOM, HREW is a little cheaper though.  Either way, you want to make sure you're using American made steel.  You're probably going to look at using 1.5-2"x1/4" wall or so, or thicker.

1/4" (0.250) wall seems a little thick and super heavy. Its good for links and sliders but most cages and sliders are 1/8" (.120) wall, thats also what most sliders and bumpers are made of on the market.

Strength tends to come from the design of a structure not necessarily the individual components.  .250 wall tube is overkill in almost any automotive application, and the weight quickly overcomes any benefit gained by the thickness of the tube.

As to costs of an internal cage, as mentioned most shops/home fabricators charge buy the hour, $80-100 seems about right.  For a one off design figure about 5hrs, plus the cost of material.  At $395, Marlin's kit is a bargain for anyone who can't fabricate their own, or doesn't have a friend who will do it for some beer.

The only downside to an internal cage is that while it saves the occupants in a roll-over, the cab itself is usually destroyed.   A buddy had a family cage put in his Runner, rolled it and now has windows/doors that wont seal up in the winter.  If you aren't in the position to swap out a cab/or buy another truck, you might want to consider an exo-cage instead.
