I bought a truck and somebody put the timing chain on wrong with the cam gear off 180 degrees from the crank.  What should I do?

The valves are bent, pull head fix

valves are fine, fix timing chain

valves may/may not be fine, fix timing chain and see

valves may/may not be fine, pull head and fix everything

If the PO didn't know what he was doing, and only went by the timing marks, the engine could have been 180* out when he removed the old t-chain.

The appearance of not being opened up doesn't necessarily mean it hasn't been, not everyone will clean engine components before rebuilding it, especially if they are in a hurry.
