Dollars to donuts they were loose since you broke 4  ;)  It pays to be anal about vehicle maintenance, especially when a vehicle sees heavy or extreme use.  I work at a busy tire shop and even if we just change 1 tire on a rig we check torque on all 4, and it's not at all uncommon to come across a few loose lugs.
do you allways tighten the wheels on the trucks with a tourqe wrench or do you just tighen it with an air gun?
