ROKTOY is right, this is the Inchworm Lefty.

Inchworm simply rotated the #4 housing to the left side, retaining all internal components including the oil galleys (flipped and mounted upright).

So everything functions just the same as a stock right-hand drop transfer case, just it's a left-hand drop.
