So I talked to people here in chat and looked online at some transmission diagrams.  It looks like the throwout bearing or pilot bearing on the clutch is having problems.

Sigh.  Is it common for them to last 5 months?   What generally causes problems like this - so that I can keep it from happening again in 5-6 months?
